ACT Supervisor - Kalamazoo
Provide active, cohesive leadership to ensure the team is providing the highest level of ACT support to the individuals being served on the team. Provide ongoing education and support to the team members.
Facilitate daily team meetings including consultations with assigned Psychiatrist.
Provide scheduled, direct clinical supervision to team staff.
Ensure team’s coordination with all other systems that may be involved—especially related to hospital or other crisis programs.
Ensure completion of required ACT and IDDT program evaluation activities.
Ensure staff completion of required documentation, assessments, intakes, and person centered plans, etc. on electronic health record.
Provide direct services, including case management, as part of the ACT/IDDT Team.
Assess mental health, substance use, and other status of persons served during daily contacts.
Provide education and support to family members, significant others, and community representatives to create a recovery network for persons served.
Monitor use of medications and effects on target symptoms.
Complete necessary paperwork to help individuals obtain and maintain income, housing, food, clothing and other benefits/services.
Develop, follow, and respect individuals’ person-centered treatment plans.
Resolve crisis situations
Participate in team’s regular on‑call schedule.
Provide back‑up assistance and consultation as needed to other staff in times of crisis.
Master's degree with 3rd party billing licensure required. (LMSW, LPC, etc.).
Minimum 2 years’ experience working with people with chronic and persistent mental illness and substance abuse issues.
Previous supervisory experience and training strongly preferred.
Effective communication, motivation, and organizational skills required.
Valid Michigan driver's license and a vehicle to transport.
Ability to respond to on-call issues within a 30 mile radius of Kalamazoo hospitals.
Ability to perform work in community settings which may not be barrier-free.
