Consumer-Centered Mental Health Service Coordination

CTT Case Management services are designed to address the needs of individuals who have experienced
the chronic problems and symptoms of mental illness and who want goal-oriented, individualized supports to
maximize their recovery. Case Management services are primarily provided in an outreach fashion to individuals
in their natural environments.
CTT staff partner with individuals to understand their illnesses and build on their strengths to reach their
personal recovery goals. Program components include:
- qualified, individual case manager
- backup from other CTT staff in times of absence or daytime emergencies
- assessment and person-centered planning to achieve maximum level of functioning
- medication monitoring and training for increased independence
- assistance with obtaining and maintaining benefits
- education about mental illness, medications, skill development
- family support and education
- coordination of, or assistance with, transportation
- assistance with obtaining and maintaining safe housing
- budgeting assistance which may include coordination with representative payee services such as
those available through InterAct's Community Payee Services
- supportive counseling
- referrals to community supports such as social, recreational, vocational, or educational
- coordination services for individuals who are hospitalized or in jail.
CTT uses a person-centered approach to assist people with planning their
treatment goals and identifying needed supports. The goal is to promote
involvement in the community, honor individuals' preferences, choices
and abilities as well as involve the family members, friends, or professionals
that consumers have chosen. An annual treatment plan is developed with
scheduled reviews to adjust services as needed.
CTT consumers receive psychiatric services through an on-site medication coordinated
by CTT's Registered Nurses. The case managers coordinate care with
the medication clinic to offer the following:
- Psychiatric services provided by a licensed psychiatrist that include assessment,
medication prescriptions and monitoring, assistance with treatment planning,
coordination with other medical professionals, and crisis consultation.
- Nursing services provided by Registered Nurses that include health assessment,
management of medication orders/re-orders, administering injections if needed,
coordination with other health care professionals, teaching and supporting
individuals to maximize their self-monitoring and success in the community.
In addition to helping people meet their personal goals, specific program-wide
outcomes for CTT case management services include:
- reduced need for psychiatric hospitalization
- increased involvement in vocationally directed activities
- maximized independent living
- high rate of satisfaction with services
