
ACT Clinician - Kalamazoo
Provide effective, recovery-oriented direct services to ACT/IDDT participants.
- Client mental health assessments, person-centered plans, daily notes and other documents using electronic health record
- Client medication monitoring
- Assist clients with case management items such as obtaining income, housing, food and other items to assure basic needs are met
- Client counseling
- Coordinate and monitor services provided within the mental health system
- Crisis resolution services
- Provide on-call services on rotating basis for Team
- Participate in daily Team meetings
- Tasks assigned by supervisor
MINIMUM REQUIREMENTS:
- Bachelor's or master’s degree with credential (LMSW, LLMSW, LPC, LLPC, LBSW, LBSW). If pending, credential must be obtained within three (3) months of hire.
- Experience and desire to work with individuals with chronic and persistent mental illness and/or substance use disorders
- Ability to perform work in community settings which may not be barrier-free
- Valid driver’s license and have own vehicle to use in the community
